A lot can be missed during the short office visit to the doctor. When a Life Care Assistant attends we can follow up with care requested by physician. We also help uncover unused benefits, VA, optimal Medicare coverage, and optimal Medi-cal coverage.

Our care managers are based out of Monterey, CA. We do consultations and assistance to seniors to transition safely from the hospital or skilled nursing settings back to home, as well as on-going care management services were you can be the loved one and we can be the help you need.
